Ascendant Health has a team of skilled nurse case managers to navigate the healthcare system, prevent medical issues, provide choices, tailor treatments, and manage care complexities. Ascendant Health leads with innovative ideas, patient choice, great provider network and promoting awareness of traumatic brain injury effects.

Role Description

This is a full-time hybrid Nurse Case Manager role located in Grand Rapids area, with some flexibility for remote work. The Nurse Case Manager will navigate healthcare systems, prevent medical issues, provide tailored treatments, and manage care complexities for patients with chronic conditions.

Qualifications

  • Strong communication and interpersonal skills
  • Excellent organizational and time-management abilities
  • Ability to work independently and as part of a team
  • Knowledge of healthcare regulations and case management processes
  • We can provide training and do pay for training as well
