Job Description

Job Title: Family Case Manager - Residential Services

Type: Direct Hire (Permanent position after 3 weeks of remote training)

Location: North Philadelphia

Hours: Monday to Friday, 9 AM to 5 PM

Start Date: ASAP (Next training session begins May 20th-June 5th)

Job Responsibilities:

  • Provide direct service links for facility residents and/or their children to external resources necessary to establish safe and more permanent housing and support.
  • Manage a caseload of clients, offering individual case management, advocacy, needs assessment, action planning, and evaluation.
  • Accompany residents and/or their children to outside appointments as needed for support and advocacy.
  • Develop, implement, coordinate, and facilitate group meetings addressing personal and emotional needs of children and residents (current and former).
  • Offer ongoing individual support and informal counseling through home visits and phone calls, making outside referrals when appropriate.
  • Identify new resources, develop informational materials, and network with community resources on women's health, housing, financial literacy, domestic violence, parenting, children's health, and other relevant topics.
  • Register/transfer children to area schools and visit schools as needed to address academic, attendance, and/or behavioral concerns.
  • Conduct intake interviews upon entry and psychosocial assessments within required timelines.
  • Provide training and education on issues such as domestic violence, parenting, and healthcare.
  • Perform exit interviews, collect and report data, and maintain professionalism, sensitivity, and confidentiality at all times.
  • Attend staff, program, and agency meetings, as well as supervision as required.

Job Qualifications:

  • Bachelor's degree in social services or a related field with some experience in direct service and/or advocacy.
  • Ability to work effectively with a diverse population.
  • Experience with domestic violence issues and bilingual proficiency are strongly preferred.
  • Prior case management experience and skills in crisis intervention, conflict resolution, group facilitation, and counseling are highly desirable.
  • Must attend upcoming Domestic Violence Advocate Virtual-Paid Training.
  • Must pass criminal (state & FBI) and child abuse clearances.
